Updated all 6 JTAs and all 6 JTA views to reflect the revised and newly aligned 30 CFR codes

Revised the 3 existing NIOSH scenarios that we decided to keep based on SME feedback (continuous miner fire, ventilation cutthroughs, and belt fire)

Updated site architecture scenario-based deliverables in Visio (available at http://msha.immersion.gmu.edu/resources#Products)
